Submit the referral form above, call (201) 781-8812, or fax patient information directly. Include the patient's insurance details and any clinical notes you have available.

Step 2 — We Respond Quickly

Our admissions team contacts you within 2 business hours to confirm receipt and discuss the patient's needs. We verify insurance benefits and begin the authorization process immediately.

Step 3 — Clinical Assessment

The patient completes a comprehensive biopsychosocial assessment with our clinical team. This can happen the same day the referral is received. We determine the appropriate level of care using ASAM criteria.

Your patient is placed in the appropriate program (PHP, IOP, Virtual IOP, or OP) and assigned to a primary clinician. An individualized treatment plan is developed. You receive confirmation of admission.
